Is This Stress, Burnout or Perimenopause?

You’re exhausted but you can’t sleep. You’re snapping at people you love. You feel anxious without being able to explain why. And somewhere underneath all of it, you’re asking yourself: what is actually wrong with me?

This is one of the things I hear most often from women who come to see me. Not always about their skin. It’s this broader feeling that something has shifted and they don’t feel like themselves anymore.

The question they’re usually asking is whether it’s stress, burnout, or perimenopause. And the honest answer is that it’s often all three at once, feeding into each other in ways that make it very hard to untangle.

Stress, burnout and perimenopause share so many of the same symptoms. That’s exactly why so many women go undiagnosed for years.

Why It’s So Hard to Tell Them Apart

Fatigue, anxiety, brain fog, mood swings, sleep disruption, low motivation, feeling overwhelmed. These are symptoms of chronic stress. They’re also symptoms of burnout. And they’re some of the most common symptoms of perimenopause.

When you’re in the middle of it, you can’t easily separate them. In many cases, you shouldn’t try to, because they’re genuinely feeding each other.

What Perimenopause Actually Is

Perimenopause is the transitional phase before menopause. It can begin in the mid-thirties. Most women don’t realise it’s happening until they’ve already been in it for a year or more.

During this time, oestrogen and progesterone fluctuate and gradually decline. These hormones don’t just affect your reproductive system. They affect your brain, your nervous system, your gut, your skin, your mood, your sleep, and your ability to handle stress.

When those hormones start to fluctuate, the effects can feel very similar to stress or burnout, because in many ways, the body is under a kind of hormonal stress.

How Stress Makes Everything Worse

Chronic stress keeps cortisol elevated. Over time, high cortisol disrupts the production of oestrogen and progesterone. So stress can worsen perimenopausal symptoms, and perimenopausal hormone changes can make you more reactive to stress. It becomes a cycle that’s hard to break.

Burnout sits at the extreme end of this. When you’ve been running on cortisol for too long, the body struggles to regulate itself. Sleep worsens. Weight shifts. Skin becomes more reactive. Mood becomes harder to manage.

None of this is weakness. It’s a physiological response to prolonged demand on a system that wasn’t designed to run at full capacity indefinitely.

Signs That Perimenopause Might Be Part of It

Changes to your cycle are often one of the first signs, periods becoming irregular, heavier or lighter, years before other symptoms appear. Skin changes are also very common: more dryness, sensitivity, or breakouts along the jawline and chin.

Other signs include joint aches, changes to libido, hair thinning, brain fog, and low mood that doesn’t quite respond to the usual things. These are often put down to ageing rather than perimenopause specifically.

What to Do If This Sounds Like You

First, stop treating it as a willpower problem. What you’re experiencing is physiological and it has real, addressable causes.

Speak to your GP about perimenopause. You may need to advocate for yourself. Many women are told it’s stress or depression before perimenopause is properly considered. Push for it to be taken seriously.

And if your skin has changed alongside everything else, that’s worth paying attention to too. The skin often reflects what’s happening internally, and understanding it can be part of building a clearer picture of what’s going on.

“The women I see in clinic are often at the point where they’ve been told everything is fine, or they’ve been managing through sheer determination for longer than is sustainable. That’s exactly who I work with.”
